Why do the hip flexors hurt with hip dysplasia?
If you have hip dysplasia, you may be familiar with that deep, aching—or sometimes sharp—pain at the front of your hip. It can feel like your hip flexors are always tight, no matter how much you stretch them.
But there may be a reason they feel that way.
In a normally developed hip, the femoral head sits securely within the acetabulum, allowing the bones, joint capsule, labrum, and surrounding muscles to share the work of keeping the hip stable.
With hip dysplasia, the acetabulum is relatively shallow and provides less coverage of the femoral head. As a result, the hip may rely more heavily on the surrounding soft tissues and muscles for stability.
One muscle that can become particularly important is the iliopsoas, one of the primary hip flexors.
The iliopsoas crosses directly in front of the hip joint. When the hip lacks normal bony stability, this muscle may work harder to help control and stabilize the joint. Over time, that increased demand can contribute to:
- Hip flexor fatigue and aching
- A persistent feeling of tightness
- Pain in the front of the hip or groin
- Irritation of the iliopsoas tendon
- Snapping or clicking sensations
This also helps explain why stretching a painful hip flexor isn't always the answer. Sometimes the muscle isn't simply "too tight." It may be working overtime because the hip is asking it to provide stability.
That distinction matters.
If your hip flexors constantly feel tight or painful despite stretching, the question may not be "How do I loosen them?"
It may be:
"Why are they working so hard in the first place?"
For someone with hip dysplasia, understanding the underlying mechanics of the hip can be an important first step toward understanding the pain.
